If it is a humiliation relationship, speak the name as if you were a prude and having to say a dirty word, or speak it as if it were a child's name, or speak it like you would call your dog (a little sharp, and a good snap of the fingers). Whether he is Ralph, Fred, Eddie, or whatever, enunciate the name in a sneering fashion. Another tack is simply to not speak his name, he is a thing and you want that thing to do this or that as ordered. It all depends on the relationship. Whether he wants to be a child, slime, or whatever, the name doesn't matter, it is how you use it to the understood purposes of your exercises.
